Claude CLI 2.1.303; Claude Agent SDK 0.3.276; Claude Opus 5.5 (`claude-opus-5-5`); Full access; default interaction; medium effort. Both runs used `/tmp/issue-24690-repro/project` and the isolated Claude config. Hook input confirms `bypassPermissions`; the existing hook returned allow without an added delay. No settings or private environment values are included.

No synthetic pending or cancelled rows were written. A cancelled approval is distinct from its successfully completed command and turn.
