Environment (primary-reported): real vp run dev web app; isolated worktree .t3; Linux x64; 1280x800; Codex 0.163.0; gpt-6.1-sol; full access. Persisted child model options independently observed: reasoningEffort=low.
Capture method: Python sqlite3.connect("file:.t3/userdata/statev2.sqlite?mode=ro", uri=True). Tables: orchestration_v2_projection_messages (text/provenance from payload_json), orchestration_v2_projection_runs, orchestration_v2_projection_subagents, corroborating projection_turn_items. Only designated parent and final attempt >=21:00Z selected. Actual thread/native IDs replaced by ROOT, A, B; run ordinals retained. No seeded conversations exported.

SCRIPTED STEPS
1. Primary opens isolated real web client at tested head.
2. At 21:00:32.495Z submits prompt below.
3. Parent launches native A/B, then ends. No wait/poll requested.
4. Allow native completion and automatic continuation; independently inspect persisted records without sending prompts or starting browser/server.

Exactly one web-created user prompt, two agent/server-created notification inputs, three completed parent runs in selected interval. No duplicate continuation in persisted state at capture. Turn items contain native children, assistant replies, notifications and checkpoints; no parent wait/poll item in selected attempt.
