No tester source edits, commits, pushes, forge writes, merges, watches, or delegation. Skills used: test-t3-app and ghdrop. Isolated worktree .t3 only; SQLite inspection mode=ro. Codex 0.163.0 / GPT-6-Astra Medium / full access. Web 7270, server 15310. Owned runner terminal session 44751.

Adapter app-server process has no CODEX_HOME override; queue uses same global home. Acceptance was not counted as consumption.

Prior history remains in disposable thread; per-run counts distinguish earlier identical READY/QUEUED/NEXT markers.

## Visual evidence and qualifications

Two final PNG frames >500ms apart differ only16 pixels in composer caret at x321,y656–671. Stable content, not pixel-identical claim. Inspected every retained image, recording contact sheet, and terminal frame. B resolved output visibly retained. Final diff against accepted before1280x800 light reflects richer history and sidebar git state, not an isolated renderer regression. No renderer changed.

Prior1440/390 light/dark files in evidence-index.md are supporting earlier-revision stills only, not final-HEAD responsive proofs. Correction to prior index: old B video resolves TOOL_23919 at18.8s; old1440-light pair differs30 composer-caret pixels, not pixel identical. This refresh closes revision qualification and final B output visibility gaps.

Unverified: explicit cancelled/late-approval execution path, approval request family, alternate providers, Electron/native mobile, relay/tunnel, full responsive/theme interaction matrix. Exact HEAD includes latest cancellation fix, but happy-path A–D is not a direct cancellation-path proof. No new tests run by this evidence-only agent.
