Successful results publish before sleeping. Delay correctly uses `min(max(old exponential backoff, 4 × elapsed), 60s)`. Carrying a `Result` preserves elapsed time on failures without rebuilding errors.\n- **Failure paths:** one shared query per polling tick prevents terminal-count multiplication. Closed/restarted terminals retain session, PID, and process identity checks. Interruption still reaches scoped ProcessRunner cleanup; timeout retains forced termination. No introduced stale-terminal update or child leak found.\n- **Compatibility/performance:** native success and custom inspectors retain zero elapsed penalty. Fast POSIX fallbacks retain existing cadence when query duration is below 250ms. Parsers, regexes, output limits, and traversal are unchanged.\n- **Sensitive diff:** no auth, credentials, permissions, approvals, runtimeMode/interactionMode, sandbox, trust, or MCP policy changes. No NEEDS_JULIUS trigger.\n\nNonblocking test gaps:\n\n- [Pacing test](/home/ubuntu/wt/pr-16874/apps/server/src/terminal/Manager.test.ts:730) checks only five seconds without another query after a five-second query. A mistaken 2× multiplier would pass. Assert no restart before 20 seconds and a restart at that boundary.\n- [Hung test](/home/ubuntu/wt/pr-16874/apps/server/src/terminal/Manager.test.ts:750) does not enforce the 15-second timeout or 60-second cap; an excessive timeout or delay could pass. Its mocked runner also cannot prove OS child cleanup.\n- A native-error → successful-fallback case would protect retained failure backoff. Gated multi-terminal close/reopen and cancellation cases would protect shared-query behavior and stale-result rejection.\n\nVerification: independently ran **47/47 focused tests**, and `git diff --check` passed. "summary": "Reviewed head [cfacceac44](https://github.com/pingdotgg/t3code/commit/cfacceac44) against `origin/main` [5a969c17e7](https://github.com/pingdotgg/t3code/commit/5a969c17e7), takeover of [#16874](https://git.cafe/pingdotgg/t3code/pulls/16874). I found no blocking bugs. One behavior change and two test gaps are worth knowing about before merging. The 47/47 focused tests pass locally.\n\n## Findings\n\n**F1 (medium, not blocking): `closeIdle` can now hold the thread lock for up to about 16s.**\n- **Where:** `apps/server/src/terminal/Manager.ts:3366`. `closeIdle` runs its own fallback query inside `withThreadLock`, separate from the shared poll.\n- **What changed:** that query used to stop at 1s (`ps`) or 1.5s (PowerShell). The new 15s limit plus ProcessRunner's 1s force-kill wait puts the worst case at about 16s.\n- **Who hits it:** the Windows fallback path, when native telemetry is down. On loaded Linux it happens only if `ps` really hangs.\n- **Failing sequence:**\n 1. The native process table is unavailable and WMI is slow or hung.\n 2. A setup script exits 0, and `ProjectSetupScriptRunner.ts:432` calls `closeIdle`.\n 3. The user opens, resizes, clears or attaches a terminal in that thread. All of these take the same lock (`Manager.ts:2884, 3052, 3211, 3214`).\n 4. They wait up to about 16s, where they used to wait about 1.5s.\n- **Pileup:** settlement runs with concurrency 8 (`ThreadSettlementService.ts:514`). Up to 8 `closeIdle` calls can each spawn PowerShell at the same time as the poll loop's query, and none of them is paced. The poll loop itself has no pileup: it is one fiber, and each query finishes before the next one is scheduled.\n- **Smallest fix:** let `closeIdle` keep a short limit (pass a per-call timeout through the fallback). Or take the snapshot and the activity marks before taking the lock, then check the marks again inside the lock.\n- **Test to add:** a hung fallback during `closeIdle` must not hold up a concurrent `resize` or `open` in the same thread for longer than the short limit.\n\n**F2 (low, test gap): no test checks the 4× pacing factor.**\n- **Where:** `Manager.test.ts`, the \"idles at least as long…\" test asserts only one query by t=11s. The query started at 1s and ended at 6s, so the next one is due at 26s.\n- **The gap:** a factor of anywhere from 1× to 4× passes. Reverting to `elapsed*1` would still pass, and so would removing the 60s cap.\n- **Fix:** advance the clock to just before 6s + 20s and expect one query, then just after and expect two. Separately, make a hung query reach the 60s cap.\n- The hung test only asserts `gap >= elapsed`, so it too accepts any factor ≥1×. That matches the note that it already passes on `main`.\n\n**F3 (low, test gap): no test covers the fast fallback path.**\n- `elapsed*4` below the 1s interval should leave the cadence at exactly 1s. Nothing asserts this on the fallback path; existing tests mostly go through the native `processTable` path with `queryElapsedMs: 0`.\n- **Failure it would catch:** a refactor using `elapsed*4 + interval` would silently slow polling on every Linux/macOS box that is on the fallback path.\n\n## Answers\n\n1. **Worth having?** Yes. Recording elapsed time with `Effect.timed` and carrying success or failure as a `Result` is about the smallest shape that does this. The old code dropped a valid but slow Windows table and polled again within about 1.5s. Values:\n - 15s query limit for both `ps` and PowerShell.\n - Next poll delay is `min(max(base·2^failures, 4·elapsed), 60s)`.\n - The 60s cap never undercuts the old exponential backoff, because that backoff had the same cap.\n2. **Concrete bugs:**\n - **Many terminals:** one shared snapshot per tick, unchanged; query rate doesn't depend on terminal count.\n - **Never returns:** bounded at 15s plus the 1s force-kill wait. The child runs inside a scope, so timeout or interruption kills it (`processRunner.ts:234-240`).\n - **Terminal closed mid-query:** the identity checks before and inside `modifyManagerState` (`Manager.ts:2599-2620`) drop results for a closed or restarted terminal.\n - **Child leaks on cancellation:** none found, for the same scoped-run reason.\n - **Stale results on the wrong terminal:** guarded by the same pid/process/session checks.\n - The only new issue is the F1 lock-hold window.\n3. **Sensitive areas:** nothing touched. No auth, credentials, permissions, approvals, runtimeMode/interactionMode, sandbox, trust or MCP changes.\n4. **Parsers, regex, timing:** the parsers are unchanged. The Windows parser is linear and splits on `|` with limit 3. The new timing math is constant cost. Timing goes through `Clock`, so it works with TestClock.\n5. **Evidence:** the matrix is a procedure spec, not a capture. Its claim that \"ps intentionally keeps 1s budget\" is outdated, because this head raises both limits to 15s. So the \"Linux 5s fake ps is NOT slow-success\" row should be re-derived. Its native-failure analysis still holds: snapshotSucceeded=false, so the failure backoff still dominates, and minute totals won't tell before from after. The first completion-to-next-start gap will. Runtime capture is still pending, and real WMI on Windows is unverified. Per the brief, I'm not rejecting on that.\n6. **Missing tests:** F1 (lock hold), F2 (4× factor and the 60s cap), F3 (fast fallback cadence). A native-fails-then-fallback-succeeds case under the new pacing would also help: it should take `max(2^n backoff, 4·elapsed)`.\n7. **Linux/macOS fast path and CPU:**\n - With native telemetry healthy, `queryElapsedMs` is 0, so the cadence is the same as before.\n - On the fallback path, a 20ms `ps` gives 80ms, which is below 1s, so it stays at 1s.\n - The quiet-tick skip still returns 0, so there is no extra wake-up.\n - The per-tick extra cost is one `Effect.timed` and one `Result` allocation, which is negligible.\n\n## Notes and verification limits\n\n- I ran `vp test run src/terminal/Manager.test.ts`: 47/47 passed. I didn't re-run the new tests on `main` to confirm \"three fail, the hung guard passes\"; I'm taking that from the brief. The hung guard passing on `main` fits the old 1.5s limit plus backoff.\n- No Windows runtime, no server launch, no browser.\n\nFRICTION: evidence|matrix.md written against the contributor head, which kept `ps` at 1s, so its Linux slow-success row contradicts this takeover|re-derive the matrix from the pinned takeover head before capturing.\n\nVERDICT: MERGE",

It touches two files: `apps/server/src/terminal/Manager.ts` and `Manager.test.ts`.\n\n**What I ran:**\n- The focused test file `Manager.test.ts`: 49/49 pass.\n- `vp run typecheck` in `apps/server`: exit 0, suggestions only.\n- `vp lint` on both files: one warning, at `Manager.test.ts:884`, which is older code (`cf9729d5ee9`), not this diff.\n- I did not run the revert experiments again. I'm relying on your numbers for those (4 fail on main, 2 fail with the closeIdle change reverted).\n\n**Gaps and notes:**\n- **Stale labels can last longer.** The poll loop never wakes early, so after a 14s query the next poll is about 56s away. A child process that has exited can keep its label (e.g. \"node\") until then, and a newly started command shows no label until then. The snapshot is taken during the query and applied after it. Before this change, a slow host got no label at all, so this is better overall. It is the trade-off the spec chose, not a bug.\n- **Slow native process tables aren't timed.** When the native table or a custom inspector is used, the reported query time is 0, so pacing ignores it. The native path has no timeout, as before. Neither is a regression.\n- **Windows is unverified.** No real Windows capture or WMI timing exists yet. The tests use a fake runner.\n\n**1. Is it worth having:** Yes. On a loaded host, every query hit the old 1s/1.5s timeout, so terminals never showed a running command. Now a slow but valid result is kept and used. The change is small: a per-call timeout, the poll result now carries how long the query took, and the delay formula takes the larger of the old backoff and 4× that time.\n\n**2. Concrete bugs:** None found.\n- **Many terminals:** there is still one shared query per tick from the single poll fiber. The query runs before the per-terminal loop, and the result is applied before the sleep.\n- **Hangs:** a hung query takes 15s plus up to 1s of forced-kill grace (`processRunner.ts:149`). `ProcessRunner` closes its scope on timeout, which kills the child, so no child process leaks. The next delay is then capped at 60s.\n- **Close and cancel:** the poll fiber lives in `workerScope`. Interrupting it closes the runner's scope, which kills `ps`/PowerShell.\n- **Wrong terminal:** `checkSubprocessActivity` still re-checks the session object, pid and process inside `modifyManagerState`. A terminal that closed or restarted during a 15s query is skipped. Polled PIDs only feed port discovery, not kills.\n- **closeIdle:** it keeps 1s on POSIX and 1.5s on Windows while holding the thread lock (`Manager.ts:3378`). A failed query still lands in the existing `Effect.catch` and is logged.\n\n**3. Sensitive areas:** None. Nothing touches auth, credentials, permissions, approvals, runtimeMode or interactionMode, sandboxing, trust or MCP.\n\n**4. Parsing and complexity:** The parsers are unchanged; the Windows snapshot function was only reformatted when it gained the timeout parameter. Moving errors into a `Result` lets successes and failures both carry the query time. That is the minimal shape for the requirement, not extra machinery.\n\n**5. The matrix is outdated on POSIX:**\n- It says POSIX keeps a 1s budget and that a 5s fake `ps` is not a slow-success case on Linux. The takeover raises POSIX to 15s too, so 5s is now a valid slow-success case on Linux.\n- A hung query now measures about 15–16s, not about 2s. The first retry is at 60s (capped), not about 8s.\n- Its formula and its revision-fence rules still hold. Real captures need these updated expectations. This does not block on its own.\n\n**6. Tests:** The new tests are meaningful.\n- Slow success is covered on Linux and Windows.\n- The pacing boundary is exact: no second query at 19,999ms, and the next query starts exactly 20,000ms after the slow one ended.\n- The hung case asserts 15s and then 60s exactly.\n- The closeIdle tests check that resize still completes within the 1s/1.5s budget.\n- Small gaps:\n - No test covers a slow query on the native-table fallback path (native fails, then a slow `ps`).\n - The fake runner doesn't model the 1s forced-kill grace, so the hung test's 60s comes from the cap, not from the measured time.\n\nNeither gap blocks.\n\n**7. Linux/macOS speed and CPU:** A fast `ps` takes about 10–50ms, so 4× that is under the 1s interval and behavior is unchanged. A slow `ps` now spends at most about 20% of its time querying and uses the same or fewer spawns per minute than the old 2→60s backoff. Nothing is pushed and nothing is implemented. One of the four tests (the hung-query one) passes on current main, which goes against the brief.\n\n**Original PR's tests.** I dropped both of [#16874](https://git.cafe/pingdotgg/t3code/pulls/16874)'s tests: \"reports activity from a slow Windows fallback and paces the next query\" and \"waits before retrying a Windows fallback that timed out\". They were Windows-only and asserted the exact 4× pacing numbers (25s and 75s). I wrote the replacements from the spec instead.\n\n**New tests.** All use `it.effect`, `TestClock.adjust` and `Deferred`. The fake `ProcessRunner` honors the caller's timeout the way `timedOutResult` mode does, and every check is an assertion, so nothing waits on a second query starting.\n1. **Slow successful query is used, Linux `ps` and Windows PowerShell:** a 5s query still produces an `activity` event labelled `node`. This covers the shared fallback path, so the implementation needs the 15s timeout on both `ps` and PowerShell.\n2. **No pile-up:** after a 5s Linux query, no second query starts for another 5s.\n3. **Hung query is bounded:** a query that never answers ends, a retry follows, and the gap before the retry is at least as long as the hung query ran.\n\n**Results:**\n\n| Run | Result |\n|---|---|\n| Baseline (main `5a969c17e7`, new tests only) | 3 failed, 1 passed |\n| PR's `Manager.ts` with ps timeout changed to 15s (temporary, restored after) | 4 passed |\n| Whole file before | 43 tests |\n| Whole file after | 47 tests |\n\nThe 3 baseline failures are assertion failures: both slow-success cases get `expected false to be true` because no activity event arrives, and the pacing test sees 3 queries where 1 was expected. None of them hang.\n\nThe hung-query test passes on main because main already ends a hung query at 1.5s and backs off 2s before retrying. Making it fail would mean asserting the exact timeout or multiplier. I kept it as a guard so the longer 15s timeout can't make a hung query unbounded. Tell me if you'd rather drop it or pin the numbers.\n\nLint and typecheck show nothing new in my lines. I didn't run the whole test file against main after the fix; the earlier full-file run timed out because of the hang described below.\n\nFRICTION:\n- test-harness | the first run hung past my 300s cap: `close` waits 1ms for processes to exit before killing them, and that wait is a sleep TestClock never advances | create TestClock managers with `processKillGraceMs: 0`, and run new tests filtered with `-t` and a hard `timeout` before the whole file\n- spec | the brief wants every new test to fail on main, but main already bounds a hung query at 1.5s | say up front which behaviors are regression guards rather than red-on-main\n- process | I spent minutes waiting on stuck background runs | run tests in the foreground with `timeout N`",

"summary": "I fixed F1 and committed it as `a77f9be73a` on `takeover/pr-16874` in `/home/ubuntu/wt/pr-16874`. Nothing is pushed. All 49 tests in `Manager.test.ts` pass, and lint and typecheck are clean.\n\n**F1: closeIdle could hold the thread lock for up to 15s.**\n- **Fix:** the fallback query timeout is now passed in on each call. It goes through `acquireSubprocessInspector` and `fetchProcessTableSnapshot` down to the ps and PowerShell snapshot functions.\n- **Poll loop:** it still passes the shared 15s timeout, so the pacing is unchanged (wait at least 4× the last query's time, capped at 60s).\n- **closeIdle:** it passes its original budget, set in `CLOSE_IDLE_QUERY_TIMEOUT`: 1s for ps and 1.5s for PowerShell.\n- **Unchanged:** the native process table and custom inspector paths ignore the timeout. ProcessRunner's scoped cancellation (the `timedOutResult` mode) is untouched. The thread lock was not refactored.\n- **Formatter churn:** `windowsProcessTableSnapshot` gained a parameter, so `vp fmt` re-indented the whole function. That makes its diff look bigger than the logic change.\n\n**Tests (47 → 49):**\n- **New regression test,** written once and run for linux and win32. closeIdle starts a query that never answers, and a resize on the same thread starts while closeIdle holds the lock. After advancing the test clock by exactly the budget (1000 or 1500 ms):\n - both fibers have finished;\n - the query lasted exactly the budget;\n - the resize reached the terminal;\n - the terminal was not killed.\n\n It uses `Deferred` and the test clock only, no real waits.\n- **Pacing test, strengthened:** a 5s query, then the clock advances to 1 ms short of the 20s wait and there is still only one query. One more millisecond and the next query starts at exactly the end of the first plus 20 000 ms.\n- **Hung-query test, strengthened:** the query must last exactly 15 000 ms and the retry must start exactly 60 000 ms after it ends.\n\n**Revert checks:**\n- With closeIdle switched back to the 15s timeout, both cases of the new test fail (2 of 49).\n- With `Manager.ts` swapped back to main's version (`5a969c17e7`), 4 tests fail: the two slow-query activity cases, the pacing test and the hung-query test. The new closeIdle test passes there, as it should, since main already used the short budget.\n\n**FRICTION:**\n- tooling | `npx tsgo` is not installed in this repo, so the first typecheck attempt fetched from the npm registry and got a 404 | use `vp run typecheck` in `apps/server`.\n- authoring | a scripted edit that turned a const into a function left `=` where `=>` was needed, which broke formatting | run `vp fmt` right after scripted edits to catch syntax slips.",
