# Issue 22925 reproduction on current main

Environment: Linux fs-28; real vp run dev; Chromium browser 1280x800, default scale; Claude Opus 5.5 parent and second Claude instance.

Attempt 1: Full-access async child waited 15 seconds then used Bash with a Python heredoc to write approval.txt. Command completed without approval. Completion woke parent normally.

Attempt 2: Same operation, async approval-required child. Parent finished at 01:37:08.217 UTC. Child live command request became pending at 01:37:55.437 UTC. At 01:39 UTC parent UI showed Subagent needs approval, but no parent run or message followed the approval.

This reproduces the missing notification when an approval exists; the reporter’s full-access trigger did not prompt in this environment. macOS Electron unverified.
