Environment: Linux fs-40; /home/ubuntu/wt/issue-25510; isolated .t3/userdata/statev2.sqlite opened mode=ro. Existing integrated run: web dev localhost:8273, Chromium Browser panel 1280x800, Claude CLI 2.1.303, claude-opus-5-5. Browser details are lead capture metadata, not independently relaunched. UTC 2026-10-26.

Sequence and resume behavior:
1. Baseline H=6af67ecf-cbeb-45ed-a7dd-cfa72fe8fdef; temporary turn then rollback to H. Session fa1a643a-72fa-406f-8d84-fe4761a035eb.
2. Before: P1 acknowledges apricot-25510 then is interrupted. P2 reopens at stale H, replies UNKNOWN. Revert P2 selects interrupted UUID 4e94edfc-c604-4772-8aa9-a7ba7c476c1c; next send fails “No message found”.
3. Valid after attempt query opens 22:53:32.610 with resume=session and resumeSessionAt=H; sessionId unset. P1 acknowledges kiwi-25510-fixed; provider turn 12 interrupted, X=001ed654-18b8-4709-82df-d21c2d539c42. Persisted nativeConversationHeadRef becomes null.
4. P2 opens 22:53:50.206 with resume=session, resumeSessionAt unset, sessionId unset; recalls kiwi. Revert P2 pins head/cursor X at 22:54:02.126. Next send opens 22:54:08.117 at X and succeeds with kiwi.
5. Revert again pins X 22:54:21.698. Immediate-stop run ordinal 15 is interrupted 22:54:26.165 before any provider turn/query opens. Head X preserved; next run ordinal 16 opens 22:54:32.212 at X and succeeds. Provider turn ordinal 15 belongs to this continuation, not stopped run ordinal 15. Do not conflate run and provider-turn ordinals.

Accuracy: runtime excerpts matched existing provider log and SQLite projections/events. Original before-runtime.txt also includes a later discarded 22:50 attempt and thinking/signatures; excluded from uploads. Earlier patched attempts completed before stop or overlapped watcher restart; excluded from valid proof. Original after-runtime.txt provider-turn ordinals are accurate; immediate-stop is an orchestration run without a provider turn.

Test proof: existing red capture is a reproduce delegate report (1 failed, 236 skipped), not raw runner output; it also reports a tightened assertion red verification. Existing green runner summary records 238 passed at 22:52:01. Evidence agent did not rerun or independently authenticate historical execution. Source reviewed: interrupted-with-output clears head and omits resumeSessionAt; separate interrupted-with-no-assistant-output retains pinned head. Runtime immediate-stop does not exercise adapter interruption without output; deterministic test covers that path.

Render matrix EMPTY: 0/0 rows. No width/theme recordings needed. Relative to merged latest main, only ClaudeAdapterV2.ts and ClaudeAdapterV2.test.ts changed (134 insertions, 1 deletion); no render changes. Server-only proof exception: red test plus runtime transcript. No claim of independent verification of other providers, clients, remote/tunnel modes, or native persistence internals beyond observed behavior.
