Issue 24221 runtime reproduction, 2026-10-24, fs-17 Linux Base: d53873b6a4 vp install: passed. vp run migrate-dev-db: failed; source records PeerLinks and PeerLinkFence at migration slots 68/69, main expects ShellStopRequestIndex and ThreadMessageSearch. Live source only read. vp run dev --no-seed: passed, isolated /home/ubuntu/wt/issue-24221/.t3, web 6637, server 14677. Browser: T3 collaborative preview, 1280x800, no device emulation. Healthy app: authenticated:true, orchestration:operate permission; existing empty persisted thread; draft 'Permission timeout reproduction — do not send.'; Submit message disabled:false. Fault: temporary AuthSessionRepository.getById Effect.tap sleeps 7 seconds when /tmp/t3-24221-delay exists. First unconditional-delay attempt restarted server and caused reconnection; inconclusive for stale permission symptom. Replaced by file-toggle fault and reconnected healthy first. Literal stale-wait/settings/return attempt did not trigger session refresh in observed interval. Deterministic variant: browser imported /src/rpc/atomRegistry.ts and /src/state/session.ts and called appAtomRegistry.refresh(environmentSession.sessionStateAtom(environmentId)) with existing healthy Success. After 6 seconds: Failure RemoteEnvironmentAuthTimeoutError timeoutMs:6000; previousSuccess retained authenticated:true and orchestration:operate. Submit disabled:true, aria-label:'This connection cannot change threads.' Draft remained intact. No message submitted; no agent started. Fault patch and toggle removed before red-test delegate. Red proof at 0c9c9f1045: vp test run src/state/commandPermissions.test.ts (packages/client-runtime) FAIL keeps a cached grant while a session recheck times out AssertionError: expected false to be true commandPermissions.test.ts:178 registry.get(permissions.permissionAtom(env)) 1 failed | 15 passed (16). Separate temporary isolation of second assertion confirmed authorize throws EnvironmentAuthorizationError: This connection requires orchestration:operate. Companion explicit rejection test passes on main.
