Environment: fs-19 Linux; real web dev app via vp run dev, Chromium collaborative preview, 1280x800.
State: isolated /home/ubuntu/wt/issue-22017/.t3; no live state writes.

Attempt 1: git init -q -b main /tmp/issue-22017-repro/empty
Add project > Local folder > /tmp/issue-22017-repro/empty.
Choose New worktree. Send: Reply with OK only. Do not run tools or change files.
Observed: Workspace preparation failed during provision worktree: Git command failed in GitVcsDriver.createWorktree (/tmp/issue-22017-repro/empty): git worktree add failed (ref_not_found).
No provider turn started.

Attempt 2: click Retry in same thread.
Observed: same workspace preparation failure, before provider turn.

Control: git init -q -b main /tmp/issue-22017-repro/committed
Create an initial empty commit. Add project. Choose New worktree. Send identical prompt.
Observed: worktree created under isolated .t3/worktrees/committed. Provider started and replied OK.

Regression proof: vp test run apps/server/src/orchestration-v2/ThreadLaunchService.test.ts -t "no commits"
AssertionError: expected 'failed' to equal 'starting'
