Issue 16441 runtime reproduction

Environment: Linux x86_64, Node 24.21.0, Chromium collaborative browser, web dev server at worktree .t3
Command: vp run dev --no-seed
Normal turn: browser submit "Reply OK. Do not use tools." -> completed, assistant reply OK.
Fault: server stopped; installed SQLite trigger on next user-message insert to add malformed running command_execution item owned by prior run.
Follow-up: browser submit "Reply OK again. Do not use tools."
